These Word forms are designed to support instructors, students, and academic integrity leads throughout each step of an academic integrity investigation or appeal. They are tailored to meet the requirements of Queen鈥檚 Academic Integrity Procedures and help users provide the appropriate level of detail.

Form required for all appeals within your faculty or school  

If you are appealing to the University Student Appeal Board, please refer to 鈥淪tarting an Appeal to USAB鈥 section on their webpage for their required form.  

Form Features 

  • Use of purple text to indicate where information should be entered. It disappears as the user types, replaced by their response in black. 
  • Embedded guidance and examples to help users understand what to write in different scenarios. 
  • Pull-down menus to customize the form for each Faculty or School. 
  • References and links to specific sections of the Academic Integrity Procedures for those who want to better understand why they are being asked to provide certain information and/or how that information may be used.  
  • Checklists to help ensure all relevant documentation is included. 

Student-Friendly Design 

  • Use of summary blocks and FAQ-style headings to help students quickly navigate the forms. 
  • Conversational 鈥渘ext step鈥 sections (e.g., preparing for a meeting) to make the process less intimidating 
  • Targeted links to the Office of the Ombudsperson鈥檚 website for additional support. 
  • Appeal form includes process guidance and tips for writing a compelling appeal.
